ILLNESS GUIDELINES

• If you’ve had a fever in the past 24 hours please wear a mask.

• Please let your injector know of your illness.

• If you are truly ill, please reschedule your appointment.

COVID GUIDELINES

• When arriving to your appointment, Wait in your car until we call to let you know you may enter.

• CLIENTS ONLY, NO GUESTS. Only bring in necessary items (keys, form of payment, phone).

• You will be asked to wash your hands upon entering.

• You will be asked screening questions and will have your temperature taken with a contactless thermometer.

• You will need to read and sign a Covid-19 consent form.

• Staff and clients will be required to wear masks.

• Please reschedule if you are feeling sick or symptomatic.
